You may register by faxing the registration form to 573-874-7597, mailing the registration to 1005 W. Worley St., Attn: Cindy Brengarth, Columbia, MO 65203, or bring the form to the Columbia Health Dept. Registration begins after 07/25/12, no registration forms will be accepted prior to this date. You will be contacted to confirm your registration and to collect the $25 fee prior to the training date.
